Death Benefit Information
The Illinois Line of Duty Compensation Act (820 ILCS 315) provides for benefits to the families of Illinois firefighters fallen in the line of duty. The Application for Death Benefits Pursuant to Line of Duty Compensation Act form should be completed and filed with the Illinois Court of Claims. If you have any questions while completing this form, please contact the Illinois Secretary of State Court of Claims Office.
Families of fallen Illinois firefighters are eligible for benefits under State law. A third party entity has created a Line of Duty Death Benefits Summary.
