The Illinois Office of the State Fire Marshal honors all fallen Illinois Firefighters, as well as the passing of a firefighter from a cause not classified as a LODD. Line of Duty Death: Reporting and Notifications

When reporting a Line of Duty Death to OSFM, please read and follow the instructions at the beginning of the notification information at the link below so we can initiate the process swiftly and completely.

Death Benefit Information

The Illinois Line of Duty Compensation Act (820 ILCS 315) provides for benefits to the families of Illinois firefighters fallen in the line of duty.  The Application for Death Benefits Pursuant to Line of Duty Compensation Act form should be completed and filed with the Illinois Court of Claims. If you have any questions while completing this form, please contact the Illinois Secretary of State Court of Claims Office

Families of fallen Illinois firefighters are eligible for benefits under State law.
