Approved and Prohibited Fireworks
Fireworks Hospital Data Sheet
The Office of the Illinois State Fire Marshal (OSFM) continues to be concerned about injuries caused each year by consumer fireworks. In the past, Illinois hospitals have provided valuable information concerning firework injuries.
OSFM is interested in determining the use patterns and subsequent injuries that may result from fireworks. We are asking for your cooperation in helping to identify firework injuries that occur. The link to the Fireworks Hospital Data Sheet is listed below. We ask that you report each firework related injury presented to your emergency room throughout the calendar year, not just during firework season. The instructions on how to submit the form are on the Data Sheet.
Thank you for your concern and continued cooperation with our efforts. If you have any questions, please contact us at: SFM.PyroFireworks@illinois.gov.
